BackgroundUmbilical cord banks are a central component, as umbilical cord tissue providers, in both medical treatment and scientific research with stem cells. But, whereas the creation of umbilical cord banks is seen as successful practice, it is perceived as a risky style of play by others. This article examines and discusses the ethical, medical and legal considerations that arise from the operation of umbilical cord banks in Mexico.DiscussionA number of experts have stated that the use of umbilical cord goes beyond (...) the mere utilization of human tissues for the purpose of treatment. This tissue is also used in research studies: genetic studies, studies to evaluate the effectiveness of new antibiotics, studies to identify new proteins, etc. Meanwhile, others claim that the law and other norms for the functioning of cord banks are not consistent and are poorly defined. Some of these critics point out that the confidentiality of donor information is handled differently in different places. The fact that private cord banks offer their services as "biological insurance" in order to obtain informed consent by promising the parents that the tissue that will be stored insures the health of their child in the future raises the issue of whether the consent is freely given or given under coercion. Another consideration that must be made in relation to privately owned cord banks has to do with the ownership of the stored umbilical cord.SummaryConflicts between moral principles and economic interests (non-moral principles) cause dilemmas in the clinical practice of umbilical cord blood storage and use especially in privately owned banks. This article presents a reflection and some of the guidelines that must be followed by umbilical cord banks in order to deal with these conflicts. This reflection is based on the fundamental notions of ethics and public health and seeks to be a contribution towards the improvement of umbilical cord banks' performance. (shrink)

Introduction Hospital delirium is a frequent, serious, costly, and underrecognized acute disorder of attention and cognition. Therefore, the prevention of hospital delirium is not only desirable for patients, caregivers, healthcare professionals, and the health system itself, but also a moral duty. Objective To identify and synthetize the main ethical aspects that arise related to the prevention of hospital delirium in patients 65 years and older. Methods A scoping review was carried out in Embase, Medline, and Web of Science. Articles published (...) in English or Spanish analyzing any pharmacologic, non-pharmacologic, or multicomponent intervention for prevention of delirium in hospitalized patients aged 65 years and over were included. Results Twenty-seven studies were included. Based on the frequency of appearance and the relevance, our findings were organized into three main areas. The first one was related to the principle of nonmaleficence and collected findings about the iatrogenic causes of hospital delirium and the use of restraints systems. The second area analyzed the impact of delirium in patients, families, and healthcare professionals from a relational ethics perspective, and addressed the family presence as a factor to prevent delirium. Finally, the third area is related to the quality of care. Conclusion Prevention of delirium in hospitals is essential to avoid the discomfort and potential clinical complications of patients, as well as the suffering of their relatives, caregivers, and healthcare professionals. Policymakers and healthcare managers are encouraged to make the required efforts to prevent delirium in hospitalized older patients. (shrink)

Background/ObjectiveSocial skills are essential in adolescence, both for their relational dimension and for their influence on other areas of adolescent life, so it is essential to include Social skills in the formal education of students.MethodThis paper presents the results of an experimental mixed factorial design pilot study in which an Interpersonal Skills Training Program for Adolescents was applied. The convenience sample consisted of 51 adolescents. An evaluation was carried out before and after the intervention, using the CEDIA and SAS-A questionnaires.ResultsThe (...) mixed factorial ANOVA show significant differences in the overall measures and in most of the subscales of both questionnaires, indicating that PEHIA is effectiveness, at least in the short term.ConclusionsThe results obtained in assertiveness, interpersonal relationships and public speaking suggest that the program is feasible and shows promising results in reducing anxiety. Since the Millennium Ecosystem Assessment in 2005, interest has increased in cultural ecosystem services (CESs) research to understand the complexity of the non-material benefits that people obtain from ecosystems. The intangible and interactive characteristics of CESs present many challenges regarding how to approach, quantify and even define CESs. In this paper, we suggest looking at CESs through the lens of embodied and situated cognition theories. We advocate that such an approach should be applied to the development stage of CES research (...) projects, as the embodied and situated experience of the site aids the development of research questions and future interventions. We described a case study—namely, the Environmental Empathy Research Challenge, which took place during the ColLaboratoire 2020 Research Residency in the Philippines. This case shows how interactive, embodied and situated workshops, such as Embodied Empathy and Walking Maps, contributed to developing a research proposal and a novel research framework, ecological embodied cognition (EEC). EEC focuses on the concept of environmental empathy to redefine the human-environment relationship. Further, based on an example of a participatory research activity, Sensing-Playing-Moving, we examined how interventions founded upon EEC principles enhance environmental empathy. (shrink)

While outcomes for children with T-cell acute lymphoblastic leukemia have improved dramatically, survival rates for patients with relapsed/refractory disease remain dismal. Prior studies indicate that glucocorticoid resistance is more common than resistance to other chemotherapies at relapse. In addition, failure to clear peripheral blasts during a prednisone prophase correlates with an elevated risk of relapse in newly diagnosed patients. Here we show that intrinsic GC resistance is present at diagnosis in early thymic precursor T-ALLs as well as in a subset (...) of non-ETP T-ALLs. GC-resistant non-ETP T-ALLs are characterized by strong induction of JAK/STAT signaling in response to interleukin-7 stimulation. Removing IL7 or inhibiting JAK/STAT signaling sensitizes these T-ALLs, and a subset of ETP T-ALLs, to GCs. The combination of the GC dexamethasone and the JAK1/2 inhibitor ruxolitinib altered the balance between pro- and anti-apoptotic factors in samples with IL7-dependent GC resistance, but not in samples with IL7-independent GC resistance. Together, these data suggest that the addition of ruxolitinib or other inhibitors of IL7 receptor/JAK/STAT signaling may enhance the efficacy of GCs in a biologically defined subset of T-ALL.Leukemia advance online publication, 30 May 2017; doi:10.1038/leu.2017.136. Justo antes de la pandemia por COVID-19, la Organización Mundial de la Salud definió unas prioridades de trabajo para la década 2020-2030. Un grupo interdisciplinario de profesionales de la salud reflexiona sobre estas prioridades, determinando unas categorías de análisis y, desde una perspectiva bioética, analiza cada una de ellas, ve su pertinencia, algunos eventos causales, las implicaciones que pueden tener si no son enfrentadas adecuadamente y hace sugerencias sobre la forma de llevarlas a cabo. En esta primera entrega se analiza (...) el primer grupo de prioridades estratégicas: “poblaciones más sanas”. En las conclusiones se destaca la conveniencia de acometer cada una de ellas de una manera colaborativa, gubernamental y privada; de reestructurar el sector salud con base en estas prioridades, aprovechando la experiencia obtenida en la lucha contra el COVID-19. La relación del ser humano con el medio ambiente también debe cambiar, apelando a la responsabilidad humana por el cuidado del planeta. (shrink)

A partir de una encuesta por correo electrónico a estudiantes de Ingeniería en el SAI de la UAMA en un contexto de enseñanza remota forzado por la pandemia, se caracteriza la gestión de la docencia, desde la visión de los estudiantes, se realiza un diagnóstico de los aspectos relacionados. Es un estudio transversal con un análisis cualitativo y descriptivo de las respuestas. La gestión docente se ve afectada por factores inherentes al proceso, así como también por factores externos. Los resultados (...) obtenidos muestran que fue impactada fuertemente al realizarse el proceso totalmente vía remota especialmente, la comunicación e interacción alumno-profesor. (shrink)

Fluid neural networks can be used as a theoretical framework for a wide range of complex systems as social insects. In this article we show that collective logical gates can be built in such a way that complex computation can be possible by means of the interplay between local interactions and the collective creation of a global field. This is exemplified by a NOR gate. Some general implications for ant societies are outlined. ©.

RESUMEN El presente texto es el resultado de un estudio dirigido a demostrar la importancia del sistema muscular en mujeres operadas de cáncer de mama, enfermedad con elevada incidencia en Cuba y el mundo. Se investigó en los principales reportes anatómicos. Se partió de la revisión documental de textos básicos para la carrera de Medicina inherentes a la asignatura Anatomía Humana. Se corroboró que autores como M. Prives, N Lisenkov y V Bushkovich y R. D. Sinelnikov; describen los músculos, pero (...) no relatan su importancia en la práctica quirúrgica. Se emplearon los buscadores en Internet y otros sitios webs de información científica, que permitieron corroborar desde un enfoque de ciencia, tecnología y sociedad la importancia del tejido muscular y su aplicación en técnicas reconstructivas en pacientes operadas de cáncer de mama. Se constató que los músculos más usados son los rectos abdominales, los dorsales anchos, los gráciles y el tejido adiposo del pliegue glúteo. ABSTRACT The present text is the result of a study directed to demonstrate the importance of the muscular system in women who had a surgery on breast cancer, illness with high incidence in Cuba and the world. It was investigated in the principal anatomical reports. It began with the documentary review of basic texts for the career of Medicine inherent to the subject Human Anatomy. It was corroborated that authors like M. Prives, N Lisenkov and V Bushkovich and R. D. Sinelnikov; they describe the muscles, but they do not report their importance in surgical practice. It was used the searche engines on Internet and other web sites of scientific information, which allowed to corroborate from an approach of science, technology and society the importance of the muscular tissue andits application in reconstructive techniques in patients who undergoa breast cancer surgery. It was stated that the most used muscles are the abdominal rectus, the latissimusdorsi, the graceful ones and the adipose tissue of the gluteus fold. (shrink)

El tema central de esta tesis de maestría en filosofía de la ciencia es el de la naturaleza de la persistencia a través del tiempo, en el contexto de nuestras mejores teorías físicas acerca del espacio-tiempo. Este problema involucra uno de los conceptos centrales en nuestra comprensión científica del mundo físico, a saber, el concepto de tiempo. La forma usual en que se plantea el problema de la persistencia involucra ciertas nociones que están conectadas con la física, pero estos supuestos (...) rara vez suelen ser formulados de manera explícita, empero, extrapolar las disputas filosóficas en torno a la naturaleza de la persistencia al contexto de espacios newtonianos, galileanos y relativistas (minkowski) no es una tarea trivial. Estos espacios son algunos de los espacios fundamentales donde ocurre la dinámica de los objetos físicos en física clásica. -/- En esta investigación he perseguido diferentes objetivos que vinculan el problema de la persistencia, las diferentes tesis que buscan resolver el problema y las estructuras espacio-temporales postuladas nuestras mejores teorías físicas: -/- 1. Articular el problema de la persistencia, así como varios supuestos de metafísica y filosofía de la ciencia indispensables para plantear la discusión, así como para formular mi solución. 2. Introducir a detalle las diversas estructuras-espacio temporales que, en nuestras mejores teorías de física, albergan a los objetos materiales persistentes. 3. Introducir herramientas formales que permitan analizar la relación de todo-parte en contextos temporales (mereología), así como las propiedades formales de la relación de ubicación que mantienen los objetos materiales con las regiones espacio-temporales. 4. Reformular las posiciones, con estas herramientas, en las estructuras espacio-temporales previamente mencionadas. 5. Ofrecer una serie de argumentos que favorecen lo que más adelante caracterizo como “perdurantismo de ubicación”, desde dos posiciones metafísicas: el unitismo, la tesis de que el tiempo y el espacio son aspectos de una sustancia más fundamental, el espacio-tiempo y el supersustantivismo, la tesis de que el espacio-tiempo y sus propiedades son suficientes para dar soporte a una ontología realista en el caso de nuestras mejores teorías físicas. -/- De esos cinco puntos me ocupo, en ese orden, en los cinco capítulos de esta investigación, con el capítulo V siendo mi contribución original a la literatura especializada sobre el tema. En los primeros capítulos (I, II) el enfoque es en presentar una serie de posiciones y asunciones estructurales en filosofía y física. En los capítulos intermedios (III, IV), hay también una presentación de posiciones filosóficas dominantes en la literatura (Balashov, 2010; Gibson y Pooley, 2006; Gilmore, 2008, 2014; Calosi y Fano, 2015; Saucedo, 2011) que acompañan a una discusión de elementos formales de mereología y postulados estructurales de las teorías naturalizadas de persistencia a través del tiempo. El capítulo V, que es mi contribución original a la discusión filosófica, integra todo lo discutido en los capítulos anteriores en una serie de argumentos que presento en favor del perdurantismo de ubicación. Although the view that sees proper names as referential singular terms is widely considered orthodoxy, there is a growing popularity to the view that proper names are predicates. This is partly because the orthodoxy faces two anomalies that Predicativism can solve: on the one hand, proper names can have multiple bearers. But multiple bearerhood is a problem to the idea that proper names have just one individual as referent. On the other hand, as Burge noted, proper names can have predicative (...) uses. But the view that proper names are singular terms arguably does not have the resources to deal with Burge’s cases. In this paper I argue that the Predicate View of proper names is mistaken. I first argue against the syntactic evidence used to support the view and against the predicativist’s methodology of inferring a semantic account for proper names based on incomplete syntactic data. I also show that Predicativism can neither explain the behaviour of proper names in full generality, nor claim the fundamentality of predicative names. In developing my own view, however, I accept the insight that proper names in some sense express generality. Hence I propose that proper names—albeit fundamentally singular referential terms—express generality in two senses. First, by being used as predicates, since then they are true of many individuals; and second, by being referentially related to many individuals. I respond to the problem of multiple bearerhood by proposing that proper names are polyreferential, and also explain the behaviour of proper names in light of the wider phenomenon I called category change, and show how Polyreferentialism can account for all uses of proper names. The current times call for reforms in educational processes. The Covid-19 pandemic had an unforeseen impact on the educational system in all countries. This need for change requires new pedagogies and new methods for teaching and learning. Understanding the need for change is essential for the formulation of adaptive proposals, as well as for the generation of training activities to complement the teaching curriculum. New educational practices lead to a vision of educational quality, with new approaches that allow the continuous (...) integration of knowledge and permanent interaction with the student. Families play an essential role in deceased organ procurement. As the person cannot directly communicate his or her wishes regarding donation, the family is often the only source of information regarding consent or refusal. We provide a systematic description and analysis of the different roles the family can play, and actions the family can take, in the organ procurement process across different jurisdictions and consent systems. First, families can inform or update healthcare professionals about a person’s donation wishes. Second, families (...) can authorize organ procurement in the absence of deceased’s preferences and the default is not to remove organs, and oppose donation where there is no evidence of preference but the default is to presume consent; in both cases, the decision could be based on their own wishes or what they think the deceased would have wanted. Finally, families can overrule the known wishes of the deceased, which can mean preventing donation, or permitting donation when the deceased refused it. We propose a schema of 4 levels on which to map these possible family roles: no role, witness, surrogate, and full decisional authority. We conclude by mapping different jurisdictions onto this schema to provide a more comprehensive understanding of the consent system for organ donation and some important nuances about the role of families. This classificatory model aims to account for the majority of the world’s consent systems. It provides conceptual and methodological guidance that can be useful to researchers, professionals, and policymakers involved in organ procurement. (shrink)

The goal of this paper is to propose a relational turn in healthcare professionalism, to improve the responsiveness of both healthcare professionals and organizations towards care of patients, but also professionals. To this end, it is important to stress the way in which difficult situations and vulnerability faced by professionals can have an impact on their performance of work. This article pursue two objectives. First, I focus on understanding and making visible shared vulnerability that arises in clinical settings from a (...) triple perspective: patient and family, health professionals, and institutions. Second, to address this challenge for professionalism, in this paper I articulate the term "relational centered-patient professionalism", which has two main axes. The relational approach means taking into account how the relationships among professionals, patients and institutions determine the constitution and evolution of those professional values. The focus on patient centered care is indispensable, because it is the ultimate goal pursued by the development of these professional values, and must always be at the center. (shrink)

El transhumanismo es una moda intelectual que propone la transformación de los seres humanos mediante diversas tecnologías. Expondremos brevemente los rasgos más conspicuos del TH, así como las principales críticas que se le han hecho. Pero la intención de este artículo no es entrar en esta polémica; aportaremos tan solo las claves imprescindibles para poder seguir adelante. Y una de las claves más intrigantes del TH es que, por debajo de su pátina tecno-futurista, remite a ciertas ideas filosóficas tan viejas (...) como, en apariencia, incompatibles entre sí. El TH remite al naturalismo radical, tanto como al nihilismo existencialista. La tesis que aquí se defiende es que tanto el naturalismo radical como el nihilismo existencialista son producto de las diversas oleadas del dualismo: del dualismo platónico antiguo, del dualismo cartesiano moderno. Una vez que separamos, a la manera dualista, la libertad por un lado y la naturaleza por el otro, podemos contar hasta dos, como hacen los dualistas, o quedarnos solo en uno, como hacen los existencialistas, que se paran en la libertad, y los naturalistas, que solo cuentan con la naturaleza. En cualquier caso, la imagen del ser humano, que es libertad y naturaleza, sale dañada. Asoma entonces el animal aporético y enfermizo al cual hay que salvar… de sí mismo. ¿Cómo? Siguiendo el método de Procusto, pero ahora con los prefijos bio e info en lugar de sierra y martillo. Hasta que el pobre ser humano encaje en el lecho de la utopía que algunos visionarios han urdido. ¿Y no hay otra vía, otra forma de mejorar la vida humana que sea más respetuosa para con la humana envergadura? Quizá sí, mas para trazar esa tercera vía, entre el naturalismo radical que mutila y el nihilismo existencialista que descoyunta y estira, habría que negar de antemano el dualismo que a los dos engendra, y atenerse a la sensatez común, en línea con la tradición aristotélica, antes que a las ensoñaciones utópicas. En nuestra opinión, el concepto aristotélico de naturaleza humana nos habilita para juzgar las antropotecnias mejor que la normatividad extraída de las visiones futuristas propias del TH. (shrink)

This paper takes issue with the predicativist’s identification of proper names and common count nouns. Although Predicativism emerges precisely to account for certain syntactic facts about proper names, namely, that they behave like common count nouns on occasions, it seems clear that proper names and common count nouns have different properties, and this undermines the thesis that proper names are in fact just common count nouns. The predicativist’s strategy to bridge these differences is to postulate an unpronounced determiner to go (...) with names when they appear to function as singular terms, making them effectively a concealed determiner phrase. In this paper I revisit these differences and argue that the predicativist’s strategy is not well justified and worse, it does not help make proper names and common count nouns unified; rather, it makes proper names exceptional as count nouns. I also discuss the referentialist’s take on names qua predicates and make some suggestions about how the syntactic difference between proper names and count nouns should be understood. (shrink)

The organ donation and transplantation (ODT) system heavily relies on the willingness of individuals to donate their organs. While it is widely believed that public trust plays a crucial role in shaping donation rates, the empirical support for this assumption remains limited. In order to bridge this knowledge gap, this article takes a foundational approach by elucidating the concept of trust within the context of ODT. By examining the stakeholders involved, identifying influential factors, and mapping the intricate trust relationships among (...) trustors, trustees, and objects of trust, we aim to provide a comprehensive understanding of trust dynamics in ODT. We employ maps and graphs to illustrate the functioning of these trust relationships, enabling a visual representation of the complex interactions within the ODT system. Through this conceptual groundwork, we pave the way for future empirical research to investigate the link between trust and organ donation rates, informed by a clarified understanding of trust in ODT. This study can also provide valuable insights to inform interventions and policies aimed at enhancing organ donation rates. (shrink)

Es evidente que el progreso científico-tecnológico, el bienestar de las sociedades industrializadas y el deterioro del medio ambiente se encuentran profundamente relacionadas. Al minarse las bases de los ciudadanos, la preocupación social por lo ambiental se ha manifestado como una institución social de ese vínculo con la naturaleza demandando una nueva educación que incluya la reflexión y modificación de su relación material con el mundo.

La gestión de la pandemia de COVID-19 por parte de los países occidentales es uno de los temas más controvertidos de los estudios sociales actuales. Uno de los conceptos centrales para analizar la ordenación de la gestión de la pandemia es el de Cosmopolítica, ya que nos permite ampliar la comprensión de la política para incluir a los ciudadanos, a los seres vivos no humanos, a los elementos terrestres y al propio SARS-CoV-2. Pero como argumentamos en este artículo, la cosmopolítica (...) necesita un poder previo, una potentia generativa que emana de la disposición de los actores mencionados. A partir del ejemplo de los meses en que se desarrolló y comenzó a inocularse la vacuna contra la COVID-19, ilustraremos la propuesta cosmoética: una ética afirmativa que tiñe y permea el agenciamiento de actores que constituyen la vacuna, permitiendo su despliegue para un posterior momento cosmopolítica. De este modo, concluimos que la gestión de la pandemia necesita no sólo atender a los ciudadanos no expertos o a los actores no humanos, sino también atender a la emergencia de nuevas realidades, tiempos y espacios. En un intento de superación del idealismo y del realismo el pensamiento de Ortega sitúa su cimiento en un terreno que se quiere más radical: la vida. Es menester entonces una crítica de los viejos conceptos que no dan cuenta de esa realidad, una criba que deje paso a una nueva forma de pensamiento capaz de explicarla. Ahora bien, habida cuenta de que los conceptos filosóficos son creaciones objetivas y esa nueva realidad es inevitablemente actual, ejecutiva, ¿es posible la empresa (...) de ese "cartesianismo de la vida"? El siguiente diálogo pretende ser una aproximación a los problemas y novedades del enfoque orteguiano, por lo que es, entonces, un acercamiento al concepto ejecutivo del ser, pero también a una nueva epistemología, que dando por sentada la existencia del conocimiento se pregunta: ¿por qué necesitamos conocer? O presente artigo analisa os sentidos abstratos para a soberania do povo como pressuposto para a fixação de sentidos concretos para a democracia participativa em um Estado de Direito. A temática é abordada pelo método dedutivo, por meio de pesquisa doutrinária e de diplomas normativos. O artigo pretende responder em que medida se afigura possível estabelecer bases de racionalidade para a construção de um discurso compatível com os direitos de participação política em um Estado Democrático de Direito.
